How to Change your Name in Passport ? (After Marriage)

Women changing their Name/Surname or Both in Passport after Marriage must present the following documents with their Passport Application at the time of Appointment :

1. Photostat Copy of the Passport of your Husband ( If available).

2. A self Attested Photocopy of the Marriage Certificate issued by Registrar or Marriage OR a Joint Affidavit of Husband and Wife with Joint Photograph sworn before the Notary on a Non-Judicial Stamp Paper of minimum value (Annexure ‘D’).

How to Change your Name in Passport ? (After Re-Marriage)

Women changing their Name/Surname or Both in Passport after Re-Marriage must present the following documents with their Passport Application at the time of Appointment :

1. Divorce Deed/Death Certificate with respect to your Previous Husband.

2. The list of documents mentioned above (As in After Marriage).

How to Change your Name in Passport ? (After Divorce)

Women changing their Name/Surname or Both or Deleting the Surname of their Spouse in Passport after Divorce must present the following documents with their Passport Application at the time of Appointment :

1. Certified copy of Divorce decree.

2. Deed poll/Sworn Affidavit by a Notary on a Non-Judicial Stamp Paper of minimum value (ANNEXURE ‘E’).

How to Change your Name in Passport ? (All Other Circumstances)

All other Applicants changing their Name/Surname or Both in Passport for any other Reasons/Circumstances must present the following documents with their Passport Application at the time of Appointment :

1. Deed poll/Sworn Affidavit by a Notary on a Non-Judicial Stamp Paper of minimum value (ANNEXURE ‘E’).

2. Newspaper Advertisement in any two daily leading newspapers in Original. Note : The newspaper should be published the city of Present Address of the Applicant.

Sample of Newspaper Advertisement : ” It is for general information that I (Your Name) Son of (Your Father’s Name) residing at (Your Complete Postal Address with Pin Code) declare that I have changed my Name from (Your Previous Name) to (Your New Name) Vide Affidavit dated (Affidavit Date) sworn before Notary (Name of the Notary,Place)”

The initial process is similar to as applying for a Fresh Passport. But After you reach your PSK or RPO for your appointment and present your documents, your Passport will be released on successful Validation of your Documents. Most likely this procedure will take minimum of 1 month for the Passport to reach your Postal Address.

    I really appreciate your time and guidance!

    I had recently changed my full name including middle name ( father’s name) for personal reasons. I had got the change published in Gazette, local newspapers and got Annexure E done for the new name (change in first name and father’s name/ middle name).
    However, my passport which expired in 1998 has my first name and middle name misspelt which was unnoticed and my old passport bears a wrong name (Old name 1) Name change was also done for the old name (Old name 2) which was used in all other legal docs (and differs from that on passport). I want to get the new name and middle name issued in my renewed passport.
    How to get this issue sorted? Please guide.
    1. Name appearing in passport (old name 1) to be changed to new name alongwith change in father name (father is no more and his pension paper states only his initials. I do not have any trace of my educational documents!)
    2. My new name is reflecting in my renewed Pan, License and Electricity bill too! In this case, my address change will have issues as it is changed from old name 2 to new name (old name 2 is different from that in passport – old name 1).
    I am ready to get an ad done for the name in passport changed to new name and affidavit also. But how to get the address changed.
    I was told that it is not possible to change middle name…. what to do now?
    Please help me get out of this issue.

    My sister is divorced and planning for re-marriage.When I checked passport site it is mentioned that for name change along with Annexure E have to give advertisement in local papers.Instead can I get her name change once after re-marriage,if so still do we need to advertise on paper or can this be avoided as will submit docs like hubby passport copy,marriage certificate
    Please suggest.

  3. I have my birth certificate. But it certificate full fillup with black pen and only my name is with blue marker pen. So, I. Just want to change it with all the same and without any query for my passport application. Can I get new birth certi or do changes in current certi which is I have?

  4. i re-newed my passport in july,2016..and now i want to change my name and my wife ch name…i m not a new married….and how much fees payment..of both name changes on one passport..another quistion..they have any police inquiry on name changes?? how long procedure new name change passport coming.

    My self Pravibhai punjabhai patel. I am residing mehsana, Gujarat and my birth date is 21/07/1960.
    In my current passport my name mentioned as pravinkumar punjiram patel (12th years old, second time renew recently). And my all remaining documents issued as Pravibhai punjabhai patel. only Living Certificate with name mentioned as pravinkumar punjiram patel. Now, I want to change my name in my passport as Pravibhai punjabhai patel.

    So, please suggest procedure for name change in passport and required document. Please also suggest is there any penalty payable.

    in 1999 i got married. in that time my husband passport spouse name not mentioned the name. empty column was there. but during renewel time that spouse column, they mentioned spouse name different name not in my name. Due to this problem i can’t take my children passport. but i added my husband name correctly in my passport. Now i want to change my husband passport wife name correctly. where can i change this, what is the ptocedure, please reply me.
