How to Change your Name in Passport ? (After Marriage)

Women changing their Name/Surname or Both in Passport after Marriage must present the following documents with their Passport Application at the time of Appointment :

1. Photostat Copy of the Passport of your Husband ( If available).

2. A self Attested Photocopy of the Marriage Certificate issued by Registrar or Marriage OR a Joint Affidavit of Husband and Wife with Joint Photograph sworn before the Notary on a Non-Judicial Stamp Paper of minimum value (Annexure ‘D’).

How to Change your Name in Passport ? (After Re-Marriage)

Women changing their Name/Surname or Both in Passport after Re-Marriage must present the following documents with their Passport Application at the time of Appointment :

1. Divorce Deed/Death Certificate with respect to your Previous Husband.

2. The list of documents mentioned above (As in After Marriage).

How to Change your Name in Passport ? (After Divorce)

Women changing their Name/Surname or Both or Deleting the Surname of their Spouse in Passport after Divorce must present the following documents with their Passport Application at the time of Appointment :

1. Certified copy of Divorce decree.

2. Deed poll/Sworn Affidavit by a Notary on a Non-Judicial Stamp Paper of minimum value (ANNEXURE ‘E’).

How to Change your Name in Passport ? (All Other Circumstances)

All other Applicants changing their Name/Surname or Both in Passport for any other Reasons/Circumstances must present the following documents with their Passport Application at the time of Appointment :

1. Deed poll/Sworn Affidavit by a Notary on a Non-Judicial Stamp Paper of minimum value (ANNEXURE ‘E’).

2. Newspaper Advertisement in any two daily leading newspapers in Original. Note : The newspaper should be published the city of Present Address of the Applicant.

Sample of Newspaper Advertisement : ” It is for general information that I (Your Name) Son of (Your Father’s Name) residing at (Your Complete Postal Address with Pin Code) declare that I have changed my Name from (Your Previous Name) to (Your New Name) Vide Affidavit dated (Affidavit Date) sworn before Notary (Name of the Notary,Place)”

The initial process is similar to as applying for a Fresh Passport. But After you reach your PSK or RPO for your appointment and present your documents, your Passport will be released on successful Validation of your Documents. Most likely this procedure will take minimum of 1 month for the Passport to reach your Postal Address.
